Choose from two unique tracks

Track 1 will explore racism, inequity and Social Determinants of Health that affect the people we serve – specifically how to move from screening for racism, inequities, and Social Determinants of Health to SOLVING for the social needs that follow from them.

Track 2 will explore COVID-19’s impact on the health care delivery system, share best practices and innovations from across the country, and assist participants in making meaningful change.

Learn about disruptors in the SDoH field creating new, evidenced-based products rooted in the efficient use of the latest technology, AI and data.

Building Collaboration

Attend roundtables after each webinar to build collaboration with:

  • Other FQHCs
  • Key players in the SDoH system: hospitals; CBOs; payers; funders; government entities, and others
  • Innovators/disruptors creating new solutions rooted in AI and big data

A Disciplined Approach

Approach the social needs related to six key social determinants with a focused, results-driven framework.

Track 2

Navigate health care delivery innovations during the COVID-19 pandemic and to prepare for the Path Forward.

QI-Based Approach

Apply QI tools and processes on a specific topic, such as telehealth and team-based care, and receive feedback from QI experts.

National Collaborations

Meet and work with health center and safety net providers, staff, consumer leaders and more as you evaluate and improve.
